Rifampin (RIF)

A

Inhibits DNA- dependent RNA Polymerase
Rec for ppl who can’t tolerate INH
Bactericidal Inducer of CYP enzymes is big problem.
Resistance, hepatotoxicity, FLU-LIKE SYNDROME, red fluids (discoloration of urine, sweat, tears)

Isoniazid (INH)

A

Inhibits glycolic acid synthesis. Cell wall disruption
Bacteriostatic &Bacteriocidal Slow acetylators – greater toxicity. Resistance, rash, liver tox., bone marrow, neurotox. Add pyridoxine (Vit B6)

No enzyme induction. Renal elimination.
Tox: Hepatotoxicity, flu- like, Orange-brown fluids, vision changes, bleeding, yellow eyes and skin
