What is the purpose of a Dose Transmittal Letter?
Method of transmitting ionizing radiation exposure information from one command to another, normally via Naval Letter.
When should the Dose Transmittal Letter be forward to the gaining command?
30 days within receipt of final exposure information.
What do Dose Transmittal Letters include
- Name of command forwarding report
- Command assigned
- Individual’s name SSN, Rate, & DOB.
- Period of exposure
- Type & amount of exposure
- Total Lifetime Exposure (LTEDE)
- Type of TLD worn
- If Exposure was reported to the NDC
What is the retention requirement for Dose Transmittal Letters?
There is none, but it is highly recommended to develop a system to document dose transmittal were sent.
Purpose of a Termination Letter
Summarizing a radiation worker’s total occupation radiation exposure received during their employment.
Done via a Naval Letter.
When should Termination Letter be submitted?
30 days after receipt of the individual final exposure information OR
After the determination that the individual will no longer be monitored for exposure to ionizing radiation.
How long are Termination Letters retained for?
5 years.
What is the purpose of the embryo/fetus statement?
“To ensure proper calculation and documentation of exposure to a declared pregnant woman and her embryo or fetus during pregnancy, and to prevent exceeding chapter 4 exposure limits in P-5055”.
Is a woman obligated to clear her pregnancy?
No.
How is the final dose estimated made at the conclusion of a pregnancy?
- The TLD collected at the time of declaration will be processed sent immediately or with the control dosimeters (depending if underway or not)
- Member will be given another TLD for the rest of the period.
- at the conclusion of pregnancy, the TLD will be sent to be processed, again immediately or with the control dosimeters (depending if underway or not).
Terminational letter is only completed if what occurs?
If the worker requests for one.
How does a woman declare her pregnancy?
*An SF-600 will be completed that is dated and signed by the woman, it will be witnessed, signed and dated by the MDR.
- The SF-600 will states something to the effect of “ I hereby make the declaration I am my pregnant, my estimated date of conception is DD MM YYYY. I understand that my occupational ionizing radiation exposure will be controlled in a manner that my unborn child will not exceeded the limits posted in chapter 4 of the NAVMED P-5055”.
- This will then be placed in her record and will remain in effect until the member withdraws the declaration or is no longer pregnant.
What is the maximum issue period for a declared pregnant worker?
- 35 days
If a declared pregnant woman transfers before giving birth, what will the MDR do?
*The exposure statement will be sent along with another statement that contains how the method the dose was estimated (during the declaration) and a final dose will be forward to gaining command with 30 days of receipt of results.
What are the exposure limits for a embryo/fetus?
500 mrem for pregnancy term, not to exceed 50 mrem/month.
