How do we document & report behaviors that occurred during sessions?

Report what occurs during therapy session using objective language and reference observable behavior without infusing perceived intentions and emotions and without using mentalistic language.

Is this a good example of NORM documentation & reporting?
Suzy was highly successful during her self-help programs today, scoring at least 80% across all target skills ( teeth, shirt, pants and hands). During table activities ( sorting, FCC, fine motor) she engaged in 6 instances of aggressive behaviors ( significantly more than during previous sessions). Escape extinction was implemented as was feasible; however, she received a break on 4 of the 6 trials because the behaviors were intense enough to require response blocking and use the Interim Control Position.

Suzy had a bad day today. She initially seemed happy when we did self-help, but was more frustrated than usual and programs were not very successful. She was aggressive and needed to be held for safety reasons. By the end of the session, she seemed less frustrated but was not as happy as when I arrived.

Report other factors that might affect the client’s behavior. Give Examples.
Illness, relocation, medical changes. earlier events, sleep deprivation, etc.
What are acceptable ways to report?
Session notes, call/text BCBA, email BCBA, etc.
