Diseases of salivary glands- Benign Flashcards
Distributions of salivary gland tumors- label the percentages that the tumor presence in these major salivary glands; is it benign or malignant?
- parotid
- submandibular
- sublingual
Parotid: 50% 50%
Submandibular: 40% B; 60% M
Sublingual: 20%B; 80% M
List in order from least to greater risk for a tumor to be malignant in the following glands:
- Submandibular
- Parotid
- Sublingual
Parotid, Submandibular, Sublingual
Sublingual is LETHAL
Distributions of salivary gland tumors- label the percentages that the tumor presence in these minor salivary glands is it benign or malignant?
- Palate
- Buccal mucosa
- Upper labial mucosa
- Lower labial mucosa
- Tongue
- Floor of mouth
- Retromolar pad
- Palate: 50-50
- Buccal mucosa: 50-50
- Upper labial mucosa: 80B- 20M
- Lower labial mucosa: 15B- 85M
- Tongue: up to 90
- Floor of mouth: up to 90
- Retromolar pad: up to 90
Describe a benign neoplasm:
- slow growth
- movable
- asymptomatic
- intact mucosa
Describe a malignant neoplasm:
- rapid growth
- fixed
- symptomatic
- ulcerated
List the benign salivary gland tumors: (5)
- benign mixed tumor (pleomorphic adenoma)
- monomorphic adenoma
- Warthin’s tumor (papillary cystadenoma lymphomatosum)
- Oncocytoma
- ductal papillomas
BMWOD
- most common salivary gland neoplasm
- middle-aged females
- painless slowly growing mass
Pleomorphic adenoma
Patient presents with this painless mass. States that it appeared awhile ago and hasn’t grown much over time. It appears an enlarged tumor of the parotid gland.. Diagnose this:
pleomorphic adenoma
Patient presents with this enlarged tumor of the palate. She is a middle-aged female and states that this is painless just annoying as it has been like this for a few years. The hole seen in the image is a biopsy site. Diagnose this:
pleomorphic adenoma
Pt presents with a tumor of the upper lip. She is middle-aged and states this is painless and has been slow growing… Diagnose:
Pleopmorphic adenoma
The most common type of salivary gland tumor encountered that can be seen in a variety of locations:
pleomorphic adenoma
What tumor presents histologically with:
- variable histology
- mixture of ductal and myoepithelial elements
- encapsulated
Pleomorphic adenoma
Patient presents with a salivary gland tumor. States that it is painless and has been around for months. Upon biopsy you note that it is a mixture of ductal and myoepithelial elements and is also encapsulated. Diagnose this:
pleomorphic adenoma
Your pleomorphic adenoma an have some cool histology, this includes:
ability to make cartilage or bone
- upper lip
- middle-aged females
- painless slow growing mass
- benign
monomorphic adenoma
Benign salivary gland tumor most likely to be encountered in the upper lip:
monomorphic adenoma
Middle aged female patient presents with slow growing mass on the upper lip. She states that she is not in pain. Diagnose this:
monomorphic adenoma
MONOMORPHIC ADENOMA: MOstly in upper lip
What tumor presents histologically with:
- uniform pattern
- single layer cords of columnar or cuboidal epithelium
- may demonstrate papillary projections
- loose connective tissue stroma with prominent vascularity
- thin fibrous capsule
monomorphic adenoma
This image shows the ENTIRE histological slide looking the exact same. Notice the loos connective tissue stroma with prominent vascularity.
Monomorphic adenoma
Patient presents and says that they have been diagnosed with Warthin’s Tumor. What is another name for this?
Papillary cystadenoma lymphomatosum
- 2nd most common benign salivary gland neoplasm
- occurs MOSTLY in the parotid gland
- Appears bilaterally
- Middle-aged males (lower in black)
- Smoking
- Painless slow growing nodular mass
papillary cystadenoma lymphomatosum
Male white patient presents with a slow growing mass, and you notice its location is on the parotid gland and its bilateral growth. You notice the smell of smoke on the patient:
Papillary cystadenoma lymphomatosum
What tumor presents histologically with:
- oncocytic ductal epithelium (uniform double rows)
- lymphoid aggregate
- papillary cystic configuration
papillary cystadenoma lymphomatosum
You take a biopsy of a parotid gland tumor and notice these uniform double rows of these oncocytic cells that appear to be lining the cystic spaces. What might be a potential diagnosis?
papillary cystadenoma lymphomatosum
- rare benign salivary gland neoplasm
- females 8th decade
- majority of these tumors affect the parotid gland
- large epithelial cells, oncocytes
- painless, slow growing mass
- MULTIFOCAL
oncocytoma
Benign multifocal tumor of the parotid gland seen in 80 year old female:
oncocytoma
What tumor presents histologically with:
- large polyhedral cells abundant in granular eosinophilic cytoplasm (due to mitochondria)
- limited stoma (thin fibrovascular septa)
- lymphoid infiltrate
Oncocytoma
