can be discharged to inpatient rehabilitation or palliative bed

more complex patients with nursing needs
provides a 4 week placement - generally in a nursing home setting
long-term care needs reassessed towards end of 4 week stay

When are fast track discharges used?

A

specific pathway to discharge patients who are reaching the end of their life
health funding for around 12 weeks
specific criteria must be met - rapid deterioration
emphasis placed on preferred place of care
individuals should have DNAR and RESPECT form in place
