What is DI?
Reduced ADH causes excessive water loss
What is nephrogenic DI?
Poor kidney response to VP
What is neurogenic DI?
Hypothalamus or pituitary injury
What are symptoms of DI?
Polyuria, polydipsia, dehydration
What are signs of DI?
Dry mucosa, sunken eyes, changes to skin turgidity
How do you distinguish between nephrogenic and neurogenic?
Water deprivation test
What does a ADH suppression test show?
Neurogenic DI
What other investigations would you do?
Urine dipstick, glucose, U&E, fluid status
How do you manage cranial DI?
Rehydration, desmopressin, head MRI
What are complications of DI?
Dehydration, electrolyte imbalance
How much urine is passed in DI?
> 3L a day
What can cause cranial DI?
Idiopathic, congenital disorders, tumours, trauma
What can cause nephrogenic DI?
Inherited, low potassium, high calcium, lithium
How do you manage nephrogenic DI?
Rehydration, treat the cause, thiazide diuretic
