Action of DPP4 inhibitors
Prevents degradation of GLP-1 (increases pancreatic insulin production)
Delays gastric emptying and decreases glucagon production

Side effects of sulphonylureas?
HYPO
Weight gain
Action of metformin?
increases insulin sensitivity
increases glucose uptake to tissues (decreases blood glucose) glycolysis
decreases glyconeogenesis
Action of sulphonylurea
Inhibits K+ channel preventing K+ exocytosis and causing depolarisation of beta cells
Side effect of metformin
Lactic acidosis (renal insufficiency) GI upset
Why is metformin often held on hospital admission?
Increased risk of lactic acidosis if Contrast imaging needed

Criteria for diagnosis of diabetes
If the patient is symptomatic:
fasting glucose greater than or equal to 7.0 mmol/l
random glucose greater than or equal to 11.1 mmol/l (or after 75g oral glucose tolerance test)
If the patient is asymptomatic the above criteria apply but must be demonstrated on two separate occasions.
