A
Muscle pain
Macular rash - shawl sign (back and shoulders)
Heliotrope rash on eyelids
Nailfold erythema
Gottron’s papules - roughened red papules over knuckles, elbows and knees

Investigations for dermatomyositis
A
Raised muscle enzymes - CK and LDH
Raised ENA antibodies (anti-Jo1 and anti-Mi2)
Muscle biopsy to confirm

Management of dermatomyositis
A
Look for underlying malignancy (30%)
Prednisolone
Hydroxychloroquine or topical tacrolimus for skin disease
