Food diary Blood for IgE Skin prick > blood Patch Dietary restrictions eliminate one food at a time
How do you test for allergic eczema
Patch testing
DO NOT require allergy testing
What is the immediate reaction in allergy
Lip swelling Urticaria Facial redness and itchy Angioedema Anaphylaxis
What are late reactions in food allergy
What should children not responding to Rx get
Worsening of eczema unresponsive to Rx
GI problems
FTT
Severe generalised itch
If this happens think food allergy
Children with moderate - severe eczema not controlled by Rx should have Rx of hydrolysed milk especially if FTT / GI symptoms
What is viral exanthems
Itchy rash associated with any viral illness
Fever, malaise, headache then rash
What are common viruses causing viral exanthems
Chicken pox Measles <1 without MMR Rubella Scarlet fever Parovirus B-19 Roseala infantum
What causes nappy rash
Irritation due to prolonged wet nappies ‘Irritation dermatitis’
Type of contact dermatitis
RF Delayed changing Irritant soaps Diarrhoea Pre-term
What are the clinical features of nappy rash
Red raw rash
Ulceration around genitals
Tends to spare the groin creases
Leads to erosions and ulceration if long standing
How do you manage nappy rash
Regular change nappies Use highly absorbant Periods of no nappy Barrier cream Topical nystatin Topical steroids if severe
What are other Ddx for nappy rash
Atopic dermatitis Seborrhoeic dermatitis Psoriasis Candidia dermatitis Contact / irritant
What should child with mod-severe eczema not controlled by Rx have
Trial of hydrolysed milk
What anti-histamines are there
Chlorophenamine
Cetrizine
What is Dapsone
Ax and anti-inflammatory
