irritant contact derm
large erythematous patch, descrete geometric pattern that matches shape of whatever caused irritation (belt buckle with nickle at umbilicus)
Allergic contact derm
may appear in same locations
erythematous and edmatous with small coalescing eczematous vesicles and papules
red and swollen
can be contact and allergic
sometimes site of exposure does not correlate with exposure location
location of dermatitis provider clues for diagnosis
clothing- rubbing areas such as extremities
cosmetics- face, neck
detergents on area of that clothing
Treatment
avoid irritant
topical corticosteriods can provide symptomatic relief, best route to find cause and avoid or treat cause
change to dye/frag free laundry soaps/diapers/socks
red area around lips from licking lips - allergy to something on tongue- use vaseline or chap stick to prevent licking- lip lickers dermatitis
