What is depression?
An affective mood disorder characterised by a persistent low mood, loss of pleasure and lack of energy
What are the core symptoms of depression?
Low mood >2 weeks
Anhedonia
Decreased energy
What is anhedonia?
Lack of enjoyment/interest
What are biological/somatic symptoms of depression?
Sleep disturbances Change in appetite/weight Diurnal variation of mood Change in psychomotor activity Loss of libido
What are cognitive symptoms of depression?
Loss of self esteem Self-reproach/guilt Suicidal thoughts Loss of concentration Hopelessness
How is the severity of depression measured?
Mild
Moderate
Severe
Severe with psychotic symptoms
Using number of symptoms
How is mild depression defined?
2 core symptoms + 2 other
How is moderate depression defined?
2 core symptoms + 3/4 other
How is severe depression defined?
3 core symptoms + 4/5 other
How is depression managed biologically?
Anti-depressant
Mood stabiliser
Anti-psychotics
ECT
What is ECT?
Electroconvulsive therapy - passage of a small electrical current through the brain
How is ECT done?
Under GA
Twice a week
6-12 sessions
What are the indications for ECT?
Severe depression
- treatment resistant
- suicidal ideation
- life-threatening
Prolonged/severe mania
Catatonia
What are short term side effects of ECT?
Peripheral nerve palsies Cardiac arrhythymias Confusion Dental/oral trauma Muscular aches Status epilepticus
What are long term side effects of ECT?
Anterograde + retrograde amnesia
