What is a thrombus?
Blood clotting in the wrong place- intravascular coagulation

Rupture of an atheromatous plaque
What is an venous thrombosis usually caused by?
Combo of Virchow’s Triad- esp. stasis and hypercoagulability

Ischaemia and infarction
What can venous thrombosis result in?
Back pressure
What causes stasis of the blood?
Immobility
Long haul flights
What causes the hypercoagulability of blood?
Pregnancy
Cancer
Sepsis
What can cause vessel damage and endothelial dysfunction?
Hypertension

High cholesterol

Indwelling venous catheters

What could a DVT become?
An embolus

Unilateral limb swelling

Compression Ultrasound
What is a D-dimer?
Breakdown product of cross-linked fibrin
When is d-dimer produced?
During fibrinolysis
How do you test for D-dimer?
Using a blood test
What are some of the symptoms of PE?
Pleuritic chest pain
Dyspnoea
Haemoptysis
Tachycardia
Pleural rub on auscultation - usually due to pulmonary infarction
What are some of the symptoms of a massive PE?
Severe dyspnoea of sudden onset
Collapse
Blue lips and tongue - cyanosis
Tachycardia
Low blood pressure
Raised jugular venous pressure
Name the three steps of diagnosing PE.
- Wells test
- Blood for d-dimer
- Imaging- Isotope ventilation/perfusion scan or CT pulmonary angiogram
