A

likely SVT with alternating bundle 2/2 dig toxicity

note that bidirectional VT is also possible (but less likely since there morphology of wide-complex QRS is more c/w aberrancy)

A

presence of ANY of the following suggests VT:

Rt precordial findings consistent w/ non-HPS activation:

  • delayed intrinsicoid deflection (> 60 ms) in V1 or V2
  • Rt precordial LV activation pattern: R > 30 ms in V1 or V2

notched S in V1 or V2

monophasic R in V6

qR in V6

notched QS in V6
