Identify (4) common types of errors in aviation operations

A

(PPDE)
-Planning: flawed from the start, lack of valid assumptions, timeline overly ambitious (planned a 2 to make 2 and flight schedule gets CANX)
-Perception: Insufficient or misperceived SA. Info was not available (Threat didn’t pop up on screen). Information was available but was missed (threat did pop up, but task saturation did not see it). Info available but misunderstood.
-Decision making: Didn’t stick to the plan or plan was not understood or was briefed poorly.
-Execution: We did not fly the correct TTP. Error in the basis: knowledge and proficiency.

The TYPE of Error informs the INSTRUCTIONAL FIX

Demonstrate the ability to provide relevant, accurate, and specific instructional fixes

A

The instructional fix is informed by the type of error. IF should prevent the root cause from happening next time. Be SPECIFIC. “The fix for _______ error is _______.” Avoid hypotheticals.

Identify and communicate Debrief Focus Points, Contributing Factors, Root Causes, and Lessons Learned

A

-Debrief Focus Point (DFP): Major point the debrief lead wishes to address. Asked in question/answer format. Why did we fail out mission and not meet comm. intent?
-Contributing Factors (CF): Statements of fact. Events that contributes to the DFP; planned of unplanned actions. List in chronological order and tie to 1 of the 4 types of errors (planning, perception, decision making, execution)
-Root Cause (RC): The ONE CF, that if avoided, would have prevented the DFP.The most correct answer to the DFP
-Learning Point: Errors made that did not contribute to failure today, but may in the future. Not as much weight,
-Lessons Learned (LL): Combine the DFP, Cos, RC, and IFs into a complete thought. A summary conclusion of the situation that took place, what went wrong, what contributed, and how to fix it so we don’t do it again.

-Fact
-Analysis
-Tapes/Teach
ASTACSOP (RAT)
-Reconstruct: White board, draw it out
-Analyze: DFP process, ask why. Take a closer look.
-Teach: Instructional Fix. **Example Cannot get ahold of receivers due to VHF radio not working, and never coordinated the UHF backup freq. Will have UHF backup next time).

What is the purpose of the debrief?

A

-Identify errors and provide fixes
-Builds a more LETHAL aircrew
-Disseminates lessons to other units (Learn from my mistakes concept)
Mistakes we make are generally the result of a bad plan, poor perception, a bed decision, or lack of execution
