Crystal Arthropathy Flashcards
How are crystal deposition diseases characterised?
Deposition of mineralised material within joints and peri-articular tissue
Common crystal deposition diseases
Gout
Pseudogout
Calcific periarthritis/tendonitis
Gout crystals
Monosodium urate
Pseudogout crystals
Calcium pyrophosphate dihydrate (CPPD)
Calcific periarthritis/tendonitis crystals
Basic calcium phosphate hydroxy-apatite (BCP)
What are tophi?
Massive accumulations of uric acid
Hyperuricaemia cause in people with gout
Reduced efficiency of renal urate clearance
Causes of hyperuricaemia
Overproduction
Under excretion
Causes of hyperuricaemia (overproduction)
Malignancy Severe exfoliative psoriasis Drugs HGPRT deficiency Inborn errors of metabolism
Causes of hyperuricaemia (under excretion)
Renal impairment Hypertension Hypothyroidism Drugs Exercise, starvation, dehydration Lead poisoning
What is Lesch Nyan syndrome?
HGPRT deficiency X-linked recessive Intellectual disability Aggressive and impulsive behaviour Self mutilation Gout Renal disease
Management of gout (acute flare)
NSAIDs
Colchicine
Steroids
Management of gout (hyperuricaemia)
1st attack not treated unless; -Single attack of polyarticular gout -Tophaceous gout -Urate calculi -Renal insufficiency Treat 2nd attack within 1 year ASYMPTOMATIC NOT TREATED
Lowering uric acid
Xanthine oxidase inhibitor
Febuxostat
Uricosuric agents
Canakinumab
What is pseudogout?
When crystals form in the synovial fluid, leads to inflammation and pain
CPPD disease
