Massive crushing of muscle> release of myoglobin into circulation > gets filtered through glomerulus > precipitated in renal tubules > acute tubular necrosis
Gfr decrease > accumulation of nitrogenous waste > uremia within 2 to 3 days of trauma

Apply torniquet and gradually release > so toxic substance get released in circulation less
Hydration with iv diuretics
If oliguria develops - treat as acute renal failure
