Definition
at distal end of radius at its corticocancellous junction ( 2cm from its articulating surface ) there is dorsal displacement of distal fragment
Age
Post menopausal women ( due to osteoporosis )
Clinical features
history of trauma
Pain
Swelling
Deformity = dinner fork deformity
Restricted movement
Radial styloid process at same level or higher than ulnar styloid process
Xray
AP =
proximal shift
radial shift
radial tilt
Lateral =
Proximal shift
Dorsal shift
Dorsal tilt
Treatment
Non operative
Operative
Non operative
undisplace = Above elbow cast with flexed elbow for 6 weeks
Displaced =
closed reduction = tration and counter traction applied along the long axis of forearm , surgeon presses the distal fragent with thim to correct dorsal tilt .
surgeon pronates forearm , flexes wrist 20 degree and 20 degree ulnar deviation and applies cast from below elbow to knuckles called colles cast .
post reduction x ray done , repeated weekly
cast applied for 6 weeks
Operative
Open reduction and
plating or percutaneous pinning
External fixation
