Olfactory
Sensory
Smell- citrus/coffee, any change in smell/taste?
Cribriform plate
Levels of scent loss
Ansomia- complete, cribriform plate fracture
Parasmia- partial, infections
Hypesmia- overwhelming scent, epilepsy/migraines
Optic
Sensory
Exits optic foramen
Vision- snellen wall chart, visual fields, pupillary reflex
Snellen chart scoring
6/6 lines read, 2 letters incorrect= 6/6 (-2)
Pupillary reflex explanation
Direct response= optic nerve recieves info + occulomotor constricts pupil
No direct response= optic recieving, occulomotor not constricting
Occulomotor
H test, pupillary reflex
Exits suboccipittal fissure
Horners syndrome, sign of palsy
Trochlear
H test- specifically looking down
Exits suboribittal fissure
Diplopia
Trigeminal opthalamic
Opthalamic- sensory to scalp, forehead, nose and upper eyelid
Exits suborbittal fissure
Trigeminal mandbular
Mandibular- lower lip/teeth, ant 2/3 tongue, lat cheek/temporal region
Motor- muscles of mastication
Exits foramen ovale
Trigeminal maxillary
Maxillary- sensory, upper
jaw/teeth, lower eyelid, palate
Foramen rotundum
Abducens
Exits sub orbittal fissure
Motor
Ability to look laterally- H test
Facial
Exits internal auditory meatus
Motor- muscles of facial expressions, paralymph- produces saliva, ask about dryness in mouth
Sensory- ant 2/3 tongue
LMN v UMN Facial
L- weakness of ipsilateral facial muscles, e.g., bells palsy
U- unilateral, e.g., stroke
Vestibulocochlear
Exits internal auditory meatus
Hearing and balance- Webers (forehead), Rinnes (mastoid P)- 512 Hz
+ve= bone conduction loss
Glossopharyngeal and vagus
Gloss- supplies mouth and throat
Vagus- responisble for internal organ function + coughing and sneezing, posterior 1/3 tongue
Uvula deviation (away), swallowing test, cough, hoarse voice
Accessory
Exits foramen magnum
Traps and SCM- muscle test
+ve= weakness
Hypoglossal
Exits hypoglossal canal
Motor- tongue and throat
+ve= weakness, tongue deviation (towards)
