Remdesivir

A
  • nucleoside base analog
  • full approval for treatment
  • dosing: 200 mg IV x1 then 100 mg IV qd for 5-10 days
  • recommended for hospitalized patients with severe covid-19 and/or on supplemental o2 for 5 days
  • 3 days for outpatients at high risk for progression to severe disease

A

Dosing: 6 mg IV/PO qd for 10 days or until hospital discharge
-AE: hyperglycemia, agitation/confusion, adrenal suppression, risk of bacterial/fungal infection

Tocilizumab

A
  • Inhibits IL-6 binding to receptor > may prevent cytokine storm
  • Dosing: 8 mg/kg IV
  • AE: elevated LFTs, neutropenia, thrombocytopenia, infections
  • indicated for hospitalized pts who are receiving systemic corticosteroids and require supplemental o2

Baricitinib

A
  • Modulates JAK signaling > anti-inflammatory; prevents viral entry into cells
  • Dosing: 4 mg PO qd for 14 days or until hospital discharge
  • AE: infections, thrombosis
  • Elevated inflammatory markers
  • patients on dexa + supplemental o2
