What are Congenital Uterine Malformations?
Abnormalities resulting from differing degrees of failure of fusion of the two Mullerian ducts at 9wks
How common are they?
Common but seldom clinically significant
What is a Didelphys?
Total failure of formation of Mullerian ducts at 9wks
2 wombs, cervic & vaginas
What is Unicornate uterus?
One Mullerian duct fails
So 1 vagina, cervic & uterus to 1 ovary
What is a Hypoplastic uterus?
Small uterus
What is a Septate uterus?
1 vagina, 1 cervix, 2 uterus (due to septum down the middle)
What is a Bicornuate; Unicollis uterus?
Bicornuate: 2 uterus joined
Unicollis: 1 cervix
What is Bicornuate; Bicollis uterus?
Bicornuate: 2 uterus joined
Bicollis: 2 cervix
What is an Arcuate uterus?
Top of uterus bulges slightly down instead of up.
Remnant of the septum.
What is a Subseptate uterus?
Like an Arcuate uterus except the bulge is slightly more prominent, the septum going about 1/3 into uterus
What are the risk associated with Congenital Uterine Malformations?
Risks;
- Renal anomalies [so renal tract imaging]
- Pregnancy-related problems (25% lead to discovery of malformations)
Outline possible treatments for Congenital Uterine Malformations
- Simple septa can be resected hysteroscopically
- Rudimentary horns need removal [open/laparoscopic]
- Bicornuate uteri no longer treated surgically as complication rate too high
What is a rudimentary horn?
One duct develops better than the other, a small ‘rudimentary horn’ is formed
