CHD symptoms
heart failure, poor feeding, dyspnoea, tachycardia, hepatomegaly, acidosis
Cyanotic HD
TGA, tetralogy of Fallot
VSD signs
loud pan systolic murmur
ECG - ventricular hypertrophy and strain
CXR - Cardiomegaly
VSD course
20% close spontaneously if small, larger need surgery
ASD signs
systolic murmur ULSE
ECG - RVH and RBBB
CXR - cardiomegaly
ASD course
Variable, depends on site
Pulmonary stenosis signs
Asymptomatic, ejection systolic murmur ULSE, thrill
PDA signs
continuous murmur below left clavicle, thrill, collapsing pulse, failure to thrive
PDA course and rx
Ibuprofen, surgery at 1y if persistent
Coarctation of aorta signs
HTN, radio-fem delay, ejection systolic murmur at ULSE
ECG - LVH and strain
CXR - rib notching ‘3’ sign
Coarctation of aorta course
stent or surgery by 5y
TGA signs
cyanosis by day 1/2
TGA course
maintain PDA (prostaglandins), surgery in first few days of life
What is Tetralogy of Fallot
large VSD, overriding aorta, RVH, Pulmonary stenosis
Tetralogy of Fallot signs
Cyanosis, ejection systolic murmur ULSE
