What happens to the lactate component of hartmans solution?
In the presence of adequate liver function, lactate is metabolised to pyruvate and then either to glucose or carbon dioxide and water, with the release of bicarb in both cases
What are important adverse effects of compound sodium lactate?
- Cardiac failure by increasing LV filling beyond point of maximal contractility
- Oedema
When should you consider reducing fluid challenge volume?
- Heart failure
- Renal impairment
- Severe liver disease
Why should you be careful using hartmanns in those with liver disease?
Cannot metabolise lactate as well
How much sodium would be provided in 500 ml hartmanns solution?
66 mmol
How much potassium would be provided in 500 ml hartmann’s solution?
2.5 mmol
Why should you avoid Hartmann’s in renal impairment?
Potassium content - may precipitate hyperkalaemia
