What is TOPFA?
Termination of pregnancy for abnormality
What are the effects of too much glucose in the mother?
Baby’s insulin rises
Baby’s glucose cannot fall as mother keeps sending more glucose
Longterm high insulin and glucose = big baby
What are the risks of maternal diabetes?
Malformations
SGA babies
Intrauterine death
Neonatal hypoglycaemia
What is Chorioamnionitis?
Acute inflammation
Neutrophils present in membranes, cord and fetal plate
What is Acute Chorioamnionitis?
Ascending infection by perineal or perianal flora which ascend vagina and get into the amniotic sac
How does ascending infection present?
Mother ill or not depends on whether raised neutrophils in blood
Intrauterine death
Cerebral palsy later in life
How does ascending infection affect baby’s brain?
Neutrophils produce cytokine storm which activates brain cells and damaged by normal labour hypoxia
Do opiates cross the placenta?
Yes
What is abruption?
Separation of placenta and uterine wall
Hypoxia in baby
Antepartum haemorrhage
What are the causes of abruption?
Hypertension
Trauma
Cocaine
