What is coeliac disease?
Long-term autoimmune condition of the small intestine
T-cell response to gluten -> villous atrophy & malabsorption
What are the complications related to
Iron deficiency
Calcium & vitamin D malabsorption
Anaemia
Osteoporosis
What are complications in childhood?
Faltering growth & delayed puberty
What is the cause in 95% of cases?
HLA-DQ2 allele
How does it present?
Persistent, unexplained abdo symptoms (indigestion, diarrhoea, abdo bloating, constipation)
Fatigue
Weight loss
Mouth ulcers
How is it diagnosed?
Serological testing: IgA
What must someone ensure before the test?
Ensure they have eaten gluten-containing foods at least twise a day for the past 6 weeks
What would you see on duodenal biopsy?
Villous atrophy, intraepithelial WBC, crypt hyperplasia
How is it managed?
Gluten free diet
What is dermatitis herpetiformis?
Intensely itchy skin, small blisters & bumps
Resolves if gluten cut from the diet
