Clinical Chemistry CPC Flashcards
What abnormality of calcium is associated with depression?
Hypercalcaemia
What is a Smith’s fracture?
Radius fracturing forwards (towards the palm) - falling on a flexed wrist
What is a Colles fracture?
Falling on an outstretched palm - radius fractures backwards, away from the palm
Differentials for hypercalcaemia
Cancer
Primary hyperparathyroidism
Sarcoidosis
What is the PTH in hypercalcaemia of malignancy?
0
Zero
Which organ physiologically produces PTHrP?
Placenta - increases calcium in the mother so that the baby can take it and create a skeleton
What are the two methods by which cancer can cause hypercalcaemia?
Metastasis/invasion of bone
PTHrP
What is the main cause of primary hyperparathyroidism?
Parathyroid adenoma
Which condition is associated with multiple parathyroid adenomas?
MEN1
Clinical features of hypercalcaemia
Moans, bones, groans, stones
Can be asymptomatic
Polydipsia/polyuria
What is a sign of chronic hypercalcaemia?
Band keratopathy - band of calcium across the eye
Are calcium stones opaque or lucent?
Opaque - not transparent
Complications of hypercalcaemia
Renal stones
Pancreatitis
Peptic ulcer disease
Skeletal changes - osteitis fibrosa cystica
What type of stones are associated with Proteus mirabilis infection?
Struvite
How can hypercalicuria be medically treated?
Thiazide diuretics
