How do you identify the difference between thyroid swelling and thyroglossal swelling?
Thyroid - moves up and down on swallowing
Thyroglossal - moves up with protrusion of tongue
Why perscribing insulin what must you be careful of?
DON’T abbreviate and be careful to read properly as can get v confusing
If in doubt ask patient - they deal with condition every day
Using an insulin syringe that is marked with insulin units as opposed to ml
What is insulin mixed with in IV formula?
50 units of insulin -> 49.5ml of sodium chloride
Why must you avoid measuring thyroid levels in acutely ill patients?
Sickness can have impact on HPT axis and lead to low levels of TSH
What drug is linked with abnormal thyroid levels?
Amiodarone
How does orlistat work?
Lipase inhibitor - prevents the intestines from absorbing fat from food
What must patients be advised of being starting Orlistat?
Not to eat a diet high in fat as this can lead to sterahorrea
When must patients be discontinued off Orlistat?
After 3 months of plateauing weight
When thinking of endocrine conditions what 3 questions should be asked?
- Too much hormone?
- Too little hormone?
- Gland too big?
What is the longest period of time orlistat can be taken for?
4 years
