Ascends transurethral spread
What commonly causes the infection?
UPEC or KEEPS

How does chronic pyelonephritis present?
Recurrent episodes of infection in the kidneys
What are the triad of symptoms experienced with pyelonephritis?
Loin pain, fever and pyuria (pus containing WBCs in urine)
What do the recurrent infections of the kidney s lead to?
Scarring of the renal parenchyma leading to CKD
Chronic pyelonephritis can progress to…
End stage renal failure
What scan is used for chronic pyelonephritis?
DMSA (Dimercaptosuccinic acid) scan
How does the DMSA scan work?
Injecting radiolabelled DMSA which builds up in healthy kidney tissue not damaged tissue. Gamma cameras show areas of healthy tissue so can see where scarring is.
