arising from acute inflammation - large volume of damage, inability to remove debris, fails to resolve

Arising as a primary lesion - no acute phase, can be result of: autoimmunal disorder, material resistant to cellular digestion, exogenous substances eg. sutures, glass, endogenous substances eg. keratin, hair, necrotic tissue
