What is subjective assessment?
Subjective assessment is information from the client‘s point of view (symptoms), including feelings, perceptions, and concerns obtained through interviews. what can the patient Tell us.
What is objective assessment?
Objective assessment is data those observable and measurable (signs) obtained through observation, physical exam nation and laboratory and diagnostic testing

Basic client demographics (name, age, address)
Presenting condition (ICF health condition)
History of presenting condition
Past medical history
Medication
Social history
Goals
Examples of objective assessment
Observations- swelling in ankles, pale
Vital signs- heart rate, respiratory rate, blood pressure, oxygen saturation
Functional- transfers, sit to stand, stairs, analysis of task
Respiratory- coughing, deep breath
Neurological- balance, cognition
Musculoskeletal- range of movement
