largely natural, largely what works for the person as they engage in it they condition it
Treatment?
can pair arousal with an aversive stimulus, typically bad smell, they smell it and the arousal is gone
conditioning
trapped in the body of the wrong sex
gender dysphoria
born with multiple genital parts
intersexuality
