Precedes the occurrence of disease and is used with healthy populations. Aims to decrease the probability of disease.
Primary prevention

Involves screening to detect and treat disease in its earliest stages; before symptoms are present. Includes screening programs.
Secondary prevention
Type of disease prevention:
Attempts to reduce complications and disability from established disease. The goal is to minimize residual dysfunction and to maximize functional level.
Tertiary prevention
Refers to risk reduction strategies applied at the population level.
Health promotion
Designed for patients who are too ill to be discharged from the hospital to a traditional extended care facility or home.
Subacute care
Type of subacute care facility:
An alternative to continued hospital stays for patients with complex nursing or medical care needs (e.g. deep wound management, complicated vascular or cardiac surgery).
Transitional units
Type of subacute care facility:
These are for stable patients who require a moderate level of care (e.g. long term intravenous therapy)
General units
Type of subacute care facility:
For patients with little or no hope of recovery (e.g. ventilator dependent patients)
Chronic units
Type of subacute care facility:
For patients with medically complex conditions when recovery is expected (e.g. acute ventilator support with difficulty weaning).
Long term transitional units
Offers care to patients experiencing debilitating acute or chronic illnesses (e.g. multiple sclerosis, muscular dystrophy, surgery, trauma)
Nursing homes
