Primary intention
Little to no tissue loss
Edges approximated (with surgical incision)
Heals rapidly
Low risk of infection and minimal scarring
(Ex: Closed surgical incision w/staples)
Secondary intention
Loss of tissue
Wound edges widely separated,unapproximated (pressure or proper burn)
Increase risk of infection
Longer healing time (granulation) and scarring
Tertiary Intention
Deep
Widely separated
Closure of wounds when free of infection and edema
Spontaneous opening of a previous closed wound
Stages of wound healing
Inflammation
Proliferative
Maturation/remodeling
Eschar
Black and hinders healing and seeing stage of healing
Serous drainage
The portion of the blood (serum) that is watery and clear or slightly yellow in appearance (fluid in blisters)
Sanguineous drainage
Contains serum and red blood cells,thick and appears reddish
Serosanguineous drainage
Contains both blood and serum
Watery and looks pale and pink bc of misguide of red and clear fluid
Purosanguineous
A mixed drainage of pus and blood (newly injected wound)
Purulent drainage
The result of infection
Thick and contains wbc, tissue debris, and bacteria
Albumin levels
3.5 - 5.5 g
Dehiscence
A partial or goal rupture (separation) of sutured wound
Evisceration
A a dehiscence that involves the protrusion of visceral organs through a wound healing
Prevention of dehiscence and evisceration
Fold folded blanket or small pillow over surgical wounds when client coughs to support wound
Hemorrhage
Escape of blood drop ruptured blood vessel, slipped suture, clot dislodgment
