Covers patients general health state and physical characteristics
4 Areas of the General Survery
- Physical Appearance
- Body Structure
- Mobility & Weight
- Behavior
Physical Appearance + normal findings
Age - appears as stated age
Sex - development is appropriate for sex and age
Level of Consciousness (LOC) - AO x 4 (person, place, time, situation)
Skin color (even color tone)
Facial Features (symmetric)
Overall Appearance (no acute distress)
Body Structure + normal findings
Stature (height within normal range)
Nutrition (weight within normal limits)
Symmetry (body parts equal bilaterally and relatively proportionate)
Posture (stands erect)
Position (sits comfortably with arms relaxed at sides)
Body Build (proportionate - arm span equals height)
Mobility & Weight + normal findings
Gait (feet shoulder width apart; walk is even bilaterally; symmetrical arm swinging)
Range of Motion (ROM) (full mobility of joints)
Weight (normal BMI)
BMI scale
Underweight (BMI <18.5) Normal (BMI 18.5-24.9) Overweight (BMI 25-29.9) Obesity, Class 1 (BMI 30-34.9) Obesity, Class 2 (BMI 35-39.9) Extreme Obesity, Class 3 (BMI 40+)
Behavior + normal findings
Facial Expression (maintains eye contact; appropriate expression for conversation)
Mood/Affect (cooperative and comfortable)
