  • Pull the jaw up/anterior/forward
  • Use OPA to keep lower teeth in front of upper teeth

Oral and Nasal Pharyngeal Airway

A
  • Help keep tongue from falling back.
  • Size OPA from lips to just beyond angle of jaw.
  • NPA 7mm in adults. Spray w Neosynephrine.
    • Insert with the bevel medially. Avoid if skull fx.

Positioning and Holding the Mask

A
  • First place with bag unattached
  • Cuff presses against bone and teeth
  • If edentulous can pack guaze rolls into cheeks.
    • or roll lower lip down under bottom part of mask
  • Don’t push down but pull jaw into the mask

Single and Two Handed Mask Hold

A

Single Handed

  • E C Grip
  • 4th and 5th fingers jaw thrust.

Two Handed

  • Much more effective
  • Thenar grip is better. Thumbs pointing down and fingers 2-5 provide chin life/jaw thrust.

Ventilating

A
  • Don’t go nuts. Can cause barotrauma and stomach distention.
  • Want 10 breaths a minute over 1 to 2 seconds
  • 500 cc per breath.
  • Consider circoid cartilage pressure. NOT thyroid cart.

Trouble Shooting

A
  • BVMing is difficult.
  • Get most experienced with biggest hands
  • Is OPA/NPA being used.
  • Take mask off and redo jaw thrust.
  • Cover beard with plastic wrape
  • Pack cheeks.
