The 4 airway Algorithms?
- Main Airway algorithm.
- Crash Airway
- Difficult Airway
- Failed Airway
You MUST recognize the CONTEXT and take appropriate actions.
RECOGNITION——->ACTION
Is this a CRASH Airway?
- Unconscious?
- Near Death
- Agonal or No respirations.
Expect NO response to Laryngoscopy!
BMV Underway
Are they pulseless?
YES–>BMV or EGD during CPR/ACLS
NO–>Intubate. Skip RSI & Optimization
CICO—>FAILED AIRWAY
Attempt 1 failed BUT oxygenating.
ACTION: Succinylcholine 2mg/kg IVP
INTUBATE 2nd attempt
CICO—>FAILED AIRWAY
ACTION: INTUBATE 3rd attempt
CICO—>FAILED AIRWAY
>3 attempts by Boss—>FAILED AIRWAY
Is this a Difficult Airway?

- LEMON-ROMAN-RODS-SMART
CALL FOR ASSISTANCE
- Forced to ACT? Yes—>RSI—>Intubate
RSI creates best condition for ONE BEST ATTEMPT
CICO–>FAILED AIRWAY!!
- NOT Forced to ACT?
Not oxygenating—>FAILED AIRWAY!!
Oxygenating:
IF BMV & EGD good & Intubation predicted
THEN RSI with DOUBLE setup
- AWAKE INTUBATION
Consider VL, endoscopic, intubating LMA, Cric
Sedate & topical used. If dynamic deteriorating airway Intubate. If stable back out and consider RSI and go.
Failed Airway
Oxygenation by ANY means possible!!
No Brain death allowed!
- Call for Help
- If CICO ACT Now. EGD/Prep for Cric.
- If LMA used use an I-LMA.
