What is psychological fatigue?
state of weariness r/t decreased motivation, prolonged mental activity or boredom that occurs in situations such as chronic stress, anxiety or depression
What physiological fatigue?
caused by depletion of energy, hormones, neurotransmitters or reduced # of neural connections due to brain injury
How is brain injury impacted in fatigue?
Possible that neuronal injury either diffuse or in the brain center controlling arousal, attention and response speed causes it. areas include RAS, limbic system, basal ganglia, etc.
What is primary fatigue?
Fatigue that results directly from injury or diseases (TBI or MS)
List 4 factors that contribute to secondary fatigue
- pain
- sleep disturbance
- stress
- emotional (anxiety and depression may cause sleep disturbance)
What are the challenges of measuring fatigue?
Requires subjective self-reporting, difficult to measure objectively
Describe the coping hypothesis
In order to maintain performance on cognitively demanding tasks overtime…expend greater effort. Possibly 2/2 impairments in attention and info processing
List 4 strategies/lifestyle changes to decrease fatigue
- decrease work hours
- reduce distraction/multi-tasking
- address psychological needs
- implement strategies to prevent/manage overload and social challenges
List 3 ways sleep disturbances impact the lives of people with TBI.
- insomnia/hypersomnia
- excessive daytime sleepiness
- decrease time to fall asleep and increase night time awakenings
How common are sleep disturbances in the TBI population?
30-80%
List 3 possible causes of sleep disorders
- depression
- increase in slow wave sleep (? response to mechanical damage)
- circadian regulation of melatonin syntheses may be disrupted which decreases REM
List 4 strategies to treat daytime sleepiness and fatigue
- sleep hygiene
- possibly modafinil
- possibly bright light treatment
