Cellulitis description
Confluent erythema
Significant edema
Warm, affected area
Cellulitis treatment
- Rest and elevate
- Augmentin or Keflex (antibiotics)
- X-Ray and culture
Erysipelas
Sudden onset cellulitis
Lymphatic involvement (red streaking)
High fever, chills, myalgia, vomiting
Pathognomonic symptoms
Erysipelas
Group A strep

Treatment
- Antibiotics (Penicillin VK, Zithromax, Biaxin)
HSV description
Painful, red blisters on genitals
Small, erythematous vesicles
HSV detection methods
Tzanck smear
Can be spread with no outbreak (viral shedding)
HSV treatment
- Valcyvlovir and Famciclovir (antiviral) PRN
- Patient education
- Chronic suppressive therapy for 1 year (small amount of antiviral ea. Day then switch to PRN)
Herpes Zoster/Shingles
Red blistering rash
Erythematous macular rash
Dermatomal pattern
herpes zoster/shingles
Pathognomonic
Lesions don’t pass the midline
Pain out of proportion before the flare
Herpes zoster/shingles treatment
- Valcyclovir and Famciclovir (antiviral)
- Amitryptiline (nerve pain)
- Pain icontrol
Hutchinson’s sign
Vesicles con on side or tip of nose
Can go blind
Found in herpes zoster/shingles
Herpes varicella/chicken pox
Small, itchy bolsters
Multiple clear vesicles, crust and pustules
Erythematous base
Herpes varicella
Spread by respiratory droplets or direct contact with vesicles
Dew drop on rose petal
Inject 2 days before presentation until all crusted
Herpes varicella treatment
- Antihistamine
- Antivirals (if not normally healthy)
- Calamine lotion
