What are the 3 steps for initial management?
- 1 or 2 anti-anginal drugs
- Short-acting nitrate
- Drugs for secondary prevention of cardiovascular disease

Why offer a BB first?
Much cheaper

GTN spray
What is the 999 rule for GTN?
5 min intervals between each one
After 3 uses + pain hasn’t gone = go hospital
= possible heart attack
Describe the 3rd step
Aspirin 75mg
ACEi
Statins
What is an example of a statin?
Atorvastatin
= most common
= can be taken any time a day
What do you tell people when using GTN spray?
Use before planned exertion
Spray once then wait 5 mins
Call an ambulance after 3 doses
Sit down + hold on to something if light-headed
What are side effects of GTN spray?
Flushing
Headache
Light-headedness
Would you ever increase dose of GTN?
NO
If BB isn’t working what do you want to do?
Increase BB to max tolerated dose
Switch to alternative BB
OR change to CCB
What may be the problem with propranolol?
Non-selective
