Mechanism of Action
A
Blocks calcium channels, causing vascular smooth muscle relaxation, which lowers BP, and slowed conduction through the AV Node, to slow ventricular rates

Indications
A
- A-Fib or A-Flutter with rapid ventricular response
- Stable, narrow-complex tachycardias uncontrolled by adenosine and vagal maneuvers

Contraindications
A
- Hypotension
- CHF
- Cardiogenic shock
- VT
- WPW syndrome
- Pt receiving IV Beta Blockers

Adverse Reactions
A
- Bradycardia
- Heart block
- Hypotension
- Asystole
- N/V, dizziness
- Headache

Dose -> Adult
A
0.25mg/kg, second dose 0.35mg/kg
5-15mg/hr for maintenance

Special Considerations
A
- BP and EKG monitoring necessary
- Have calcium chloride available to reverse effects
- Can only be kept out of refrigeration for 1 month
