Sinus Dysrhthmia
Rate - 60 - 100 or slower
Rhythm - irregular
P wave - upright 1/QRS
PRI - .12 - .20
QRS - < .12
The rate changes with respirations
Wandering Atrial Pacemaker
(WAP)
Rate - 60 - 100 or slower
Rhythm - slightly irregular
P wave - non-uniform 1 per QRS
PRI - can vary less than .20
QRS - < .12
3 or more p waves have differing shapes.
Multifocal Atrial Tachycardia
Rate - greater than 100
Rhythm - irregular
P wave - non-uniform with 3 different forms
PRI - varies
QRS - < .12
Similar to WAP, but faster and may look like A-FIB
Supraventricular Tachycardia
SVT
Rate - 150 - 250
Rhythm - regular
P wave - may be flat 1 per QRS
PRI - .12 - .20
QRS - < .12
P Waves can be hidden in T Waves
Atrial Flutter
Rate - (atrial - 300) (vent - varies)
Rhythm - usually regular
P wave - saw-tooth pattern
PRI - non
QRS - < .12
Constant or variable block = 3:1 or 4:1
Atrial Fibrillation
Rate - (atrial - irregular) ( vent - varies)
Rhythm - irregular
P wave - none
PRI - none
QRS - < .12
PSVT
Rate - 150-250
Rhythm - regular
P wave - buried
PRI - none
QRS - < .12
PAC
Rate - interrupts base rhythm
Rhythm - name underlying rhythm
P wave - differs from baseline rhythm
PRI - variable
QRS - no compensatory pause. Pri varies from base rhythm
Junctional Escape
Rate - 40 - 60
Rhythm - regular
P wave - inverted or missing
PRI - shortened or absent
QRS - < .12
P wave can appear before, during, or after QRS complex.
Accelerated junctional

QRS - < .12
P Wave can appear before, during, or after the QRS
Premature Junctional Contraindication
(PJC)
Rate - interrupts base rhythm
Rhythm - name underlying rhythm
P wave - inverted or missing
PRI - shortened or absent
QRS - < .12
Possible compensatory pause with a normal QRS
