what are the risk factors for breast cancer
- Nulliparity
- First full term pregnancy after age 30
- Early menarche (before age 12)
- Late natural menopause (after age 50)
- Postmenopausal combination HRT
- Hx of uterine cancer
- Greatest risk factor - personal history of breast cancer
what is the typical presentation of a breast cancer
- painless breast mass
- Usually hard, fixed, irregular margins, nonmobile
- MC in upper outer corner!!!
can also see nipple/skin retraction
what are findings that are suggestive of advanced breast cancer
- Marked axillary LAD
- Supraclavicular or infraclavicular LAD
- Edema of ipsilateral arm
- Large primary tumor (>5 cm)
what is the typical presentation of pagets disease
pain, itching or burning of breast along with superficial erosion or ulceration
what is the diagnostic of choice for pagets disease
full thickness biopsy
what is the treatment of pagets disease
mastectomy
what is the presentation of inflammatory carcinomas of the breast
- Diffuse, brawny edema of skin with erysipeloid border
- “Peau d’orange” (orange peel skin)
- this is suspected if mastitis does not resolve within 1-2 weeks of ABX usage

surgery, radiation, chemo

at least once every 2 years among women 50-74

- ER/PR/HER2 +: metastasize to bone, soft tissue, genital organs
ER/PR/HER2-: metastasize to liver, lung, brain

- Tamoxifen or Anastrozole

pembrolizumab (Keytruda) + radiation+ chemo

- tamoxifen
- raloxifene
what are the aromatase inhibitors
- anastrazole (Arimidex)
