Bleeding disorders Flashcards
What is the genetic inheritance of haemophilia?
X linked recessive
What is haemophilia A?
Deficiency of factor VIII (8)
What is haemophilia B?
Deficiency factor IX (9)
What are the features of haemophilia?
Haemarthrosis
Haematomas
What is seen on bloods with haemophilia?
Prolonged APTT
Normal PT
What is the management of haemophilia?
Factor replacement
What is the complication of treatment in haemophilia A?
10-15% develop antibodies against factor VIII treatment
What is the genetic inheritance of von Willebran’d disease?
Autosomal dominant
(type 3 is recessive)
What is the typical presentation of von Willebrand’s?
Platelet type bleeding - epistaxis, menorrhagia, bruising
What is seen on bloods with von Willebrand’s?
Prolonged bleeding
Sometimes prolonged APTT
May have reduced factor VIII
What are the types of von Willebrand’s?
1: reduction in vWF (most common)
2: abnormal vWF
3: absence of vWF
What is the treatment for von Willebrand’s?
TXA for mild bleeding
Desmopressin
Factor VIII concentrate
How does desmopressin work in von Willebrand’s?
raised levels of vWF by inducing release from endothelial cells
