bipolar disorder
main drugs: lithium & valproic acid
drug therapy includes:
mood stabilizers, antipsychotics, & antidepressants
non drug
education, psychotherapy, ECT
Lithium levels
monitor every 2-3 days, then every 3-6 mo
therapeutic: 0.6-1.2 mEq/L
side effects of lithium
tremor, goiter, polyuria
can be teratogenic
should patients increase or decrease their sodium levels?
maintain
lithium toxicity
vomiting, diarrhea, slurred speech, twitching
diuretics and lithium
diuretics can decrease lithium excretion, thereby causing toxicity
drug interactions w/ lithium
diuretics, non-steroidal anti inflame drugs, anticholinergic
tolerance for lithium
tolerance for lithium is high in acute phase of mania and decreases as mania subsides.
Antiepileptic Drugs
Suppress mania and/or depression and stabilize mood in patients with BPD
drug: Divalproex sodium
valproate
has become a first-line treatment for BPD
valproate toxicity
thrombocytopenia, pancreatitis, and liver failure
