A
  1. Cross peptidoglycan layer and enter periplasmic space
  2. Bind with penicillin binding protien (PBP); aka Transpeptidase
  3. Inhibits cross linking and cell wall formation
  4. BACTERIACIDAL!!

Characteristics of Penicillin

A
  1. Cell wall synthesis inhibitor
  2. Highly protein boud (polar)
  3. Stays in blood longer and widely distributed
  4. Unbound drug excreted in urine

Natural Penicillin (V and G)

A

Cell wall synthesis inhibitor
Active against streptococcus
ADRS: GI,Hypersensitivity, Secondary infx
DIs: bacteriorstatic abx and ORAL Contraceptives
Synergistic activity with aminoglycoside
Penicillin V = Oral –> minor infx: strep throat, impetigo, otitis media
Penicillin G = Shot/IV –> strep, syphilis, meningococci
EMPTY STOMACH!

Penicillin with Beta-lactamase inhibitor

A

Pairing a beta-lactam w/ a B-Lactamase Inhibitor negates resistance; EFFECTIVE = maintains drug activity
Active against Strep
ADRs: GI (common), Hypersensitivity (severe), 2nd infx (likely)
i.e. Amoxicillin with Clavulanate = Augmentin
Used for more invasive bacteria

Cephalosporin

A

Cell Wall Snythesis Inhibitor
Active against gram (+) cocci
Good for ingrown toenails/cellulitis
i.e. Keflex or Cephalexin

Wide spectrum
Used mostly in hospital and ICU; given IV
***Very renal toxic

A

Seizures can result from very high doses
