What can cause an obstruction of Bartholin gland?
Trauma or infection
Are chronic Bartholin gland (cysts) a concern?
Often aseptic and benign
Patient presents with periodic painful swelling on either side of introitus, dyspareunia, fluctuant swelling 1-4cm to labium minus and tenderness, what condition might they have?
Bartholin’s gland abscess
What labs should be done on a patient with a Bartholin’s gland abscess?
Purulent discharge should be cultured for GC
What is the treatment for Bartholin’s gland abscess?
Aspiration, I&D
Frequent warm soaks
Are antibiotics necessary for Bartholin’s Gland abscess?
Unnecessary unless cellulitis is present or infection is spreading
How should asymptomatic cysts of the Bartholins gland be treated?
Under 40: do not require therapy unless problem with intercourse
Over 40: biopsy/removal recommended to rule out vulvar carcinoma
Who should patients with Bartholin’s gland abscess be referred to?
OB-GYN for reoccurrences
