B Vitamins and Iron Flashcards
What is another name for Vit B1?
thiamine
What is another name for Vit B2?
riboflavin
What is another name for Vit B3?
niacin
What is another name for Vit B6?
pyridoxine
What is another name for Vit B12?
cobalamin
Indications for B1
- Prophylaxis for deficiency
- Alcoholics
- Beriberi Syndrome
- Wernicke-Korsakoff Syndrome
Indications for B2
- deficiency
2. migraine prophylaxis
S/E for B2
yellow body fluids
Indications for B3
hypercholesterolemia
B3 MOA
increases HDL, decreases LDL and VLDL
B3 S/E
flushing, nausea
can take aspirin to prevent flushing
Indications for B6
- morning sickness/nausea
2. Isoniazid antidote (TB medication)
Indications for B12
- pernicious anemia
- intrinsic factor deficiency
- gastric bypass
- vegetarian/vegan
B12 Route of Administration
- IM for gastric bypass, pernicious anemia, intrinsic factor deficiency
- PO for vegetarian/vegan
Indications for folic acid
prenatal vitamin, megaloblastic macrocytic anemia
Folic Acid Function
prevent neural tube defects in fetuses ie. spina bifida, reduces side effects of methotrexate
What do you have to monitor in RA patients taking methotrexate?
serum transaminase levels for hepatic dysfunction –> folic acid reduces levels
Indication for iron
iron deficiency anemia
Causes of iron deficiency anemia
chronic blood loss, malnutrition, pregnancy, renal failure
What labs do you order to screen for iron deficiency?
TIBC (total iron binding capacity), ferritin, total iron, transferrin
S/E of iron
nausea, abdominal pain, black stool, constipation
What should the patient take with their iron supplement? With food or empty stomach?
Vit C - it enhances absorption
empty stomach
What inhibits iron absorption?
Antacids, PPIs, H2 antagonists, Calcium
Route of administration of iron
PO (ferrous sulfate) or IV (iron dextran)
*anaphylaxis w/ IV form
What foods have Vit B1 in them?
enriched flour and cereal
What can B1 deficiency lead to?
brain damage which manifests as Wernicke-Korsakoff Syndrome
Why are alcoholics at higher risk for B1 deficiency?
absorption is markedly decreased in alcoholics
Function of B2
plays a role in ATP production through metabolism of carbs, fats, and proteins
Function of B3
plays role in lipid and protein metabolism
Function of B6
essential for production of Vit B12 and folate, necessary for absorption and metabolism of amino acids
Function of B12
key role in DNA synthesis
Effects of Vit B12 deficiency
- large, dysfunctional RBC precursors
2. affect on nervous system - inflammation, demyelination, neuronal cell death
What secretes intrinsic factor?
parietal cells in stomach
Which is absorbed better: ferrous iron or ferric iron?
ferrous iron (from animal)
Who should be given IV iron instead of PO iron?
patients who are unable to take or tolerate oral iron
Patients which what symptoms should be tested from iron deficiency anemia?
restless leg symptoms
When is iron toxicity likely?
in patients with iron storage disease ie. hemochromatosis
and iron loading anemias ie. thalassemia
