Atropine Flashcards
Atropine: Class
- antiarrhythmic
- anticholinergic
Atropine: Action
- Competes with acetylcholine for receptor sites, blocking the PNS response at the SA and AV nodes
- Increases heart rate by increasing electrical conduction through the heart
- Positive chronotropic properties with little or no inotropic effects
- Inhibits secretions by decreasing PNS effects on bronchial, salivary, sweat and GI glands.
Atropine: Onset/Duration
Onset: IV/IO 2-4 minutes. IM= 10-15 minutes
Duration: 2-6 hours
Atropine: Indications
- Adult, Unstable Bradycardia (narrow complex)
- Adult unstable Brady WIDE complex ** if external pacemaker unavailable
- PEDS: unstable bradycardia ** after BVM for 30 seconds and 3rd dose of EPI
- Poisoning/OD: symptomatic organophosphate poisoning
Atropine: dosage/route for
Narrow complex Unstable Brady (adult)
0.5 mg IV/IO for pulse < 60
MR q 3-5” to max 3mg
Atropine: dosage/route for
Adult: Wide complex, Unstable Brady (if no external pacemaker available)
Adult:
May give atropine 0.5 mg IV/IO for pulse <60
MR q 3-5” to max 3mg
Atropine: dosage/route for
PEDS unstable Brady
After BVM for 30 seconds AND 3rd dose of EPI:
<9 years HR < 60
9-14 yrs: HR < 40….. PDC IV/IO. MR in 5 minutes
Atropine: dosage/route for
Poisoning/OD (organophosphate poisoning)
Adult:
2mg IV/IM/IO; MR x2 q 3-5” MR q 3-5” prn BHO
PEDS:
PDC IV/IO/IM; MR x2 q 3-5” MR q 3-5” prn BHO
Atropine: contraindications
Asystole
(* STEMI - use with caution (may extend infarct area)
- may worsen Bradycardia associated with Mortiz 2 and 3rd degree heart block) … “cautions and considerations”
Atropine: Side Effects
- Tachycardia/palpitations
- dry mouth/nausea/vomiting
- Pupil dilation/blurred vision
- Flushed/hot/dry skin

Atropine: Cautions and Considerations
- if STEMI, cause caution (may extend infarct area)
- May worsen bradycardia associated with Mortiz II or 3rd degree heart block
