- slows electrical conduction through AV node, and interrupts re-entry pathways…. converting SVT into Normal Sinus Rhythm (NSR)
Adenosine: Onset and Duration
Onset: within 30 seconds
Duration: 1-2 minutes
Adenosine: Indications
Adult:
- SVT
PEDS:
- unstable SVT
Adenosine: Dosage/Route (adult)
SO:
6mg rapid IV/IO ** followed WITH: rapid 20ml of Normal Saline
12mg rapid IV/IO.. followed with rapid 20ml NS
- MR x1 in 1-2” (if no sustained rhythm change)
BHO if patient has history of bronchospasm or COPD

If patient has history of bronchospasm or COPD
Adenosine: Dosage/Route - PEDS:
BHPO:
- 1st dose PDC, rapid IV ** followed WITH 20ml Normal Saline
- 2nd dose PDC, rapid IV, followed with rapid 20ml NS
- MR x1 if no sustained rhythm change (BHPO)
Adenosine: Contraindications
- 2nd and 3rd degree AV heart block
- sick sinus syndrome (without pacemaker)
Adenosine: Side Effects
- SOB/dyspnea; may cause bronchospasm in COPD patients
- chest pressure/palpitations
- facial flushing/headache
- Dizzy/lightheaded
- Nausea
- transient arrhythmia
