ATN stages lentele UW 12/13 Flashcards
3 stages?
Initiation, maintenance, recovery
Initiation - duration?
24-36h
maintenance - duration?
1-3 weeks
recovery - duration?
months
Initiation. mechanism?
tubular injury resulting from: Ischemia or toxins
Maintenance. What changes?
Oliguric renal failure - decr. GFR, decr. urine output, fluid overload
incr BUN:Cr (cia tas padidejimas del sumazejusio GRF, bet siaip tai nefunkcionuoja lasteles, tai turetu buv <15:1), incr. potassium, incr phosphate, metabolic acidosis
osmol < 350 mOsm
UNa>30, FENa >1 proc.
Recovery. What changes?
Gradual increase in urine output - leads to high duresis
continued impairment of tubular function resulting in electrolyte wasting (two arrows down to potassium, Mg, P, Ca).
Why there is poliuria?
GFR often improves BEFORE renal tubular function.
Poliuria and electrolyte wasting.
