ARTHROPATHY-DISH Flashcards
very common in the elderly, greater than 35% in the elderly
not a true arthropathy
***presents as heel pain
diffuse idiopathic skeletal hyperostosis (DISH)
this is predominantly an axial disorder affecting the foot extra articularly
diffuse idiopathic skeletal hyperostosis (DISH)
key clinical features of DISH?
large spurs
-osteoproliferative enthesopathy body wide
palpable olecranon spur (tennis elbow)
dysphagia
T/F, bamboo spine is a clinical manifestation?
t, spurring happens around the spine (enthesophytes) anterolaterally
- ossified bone across vertebral bodies
- restricts movements of spine
main appendicular manifestations?
heel and joint pain, not a cause
radiology of DISH?
bony proliferation ligament calcification para articular osteophytes new bone formation after injury exuberant midfoot spurring
calcaneal sites?
3,4,5,6
pedal locations?
calcaneal sites Dorsal talus and midfoot base of the 5th met antero medial navicular and 1st cuneiform deltoid ligament
DISH can have what look likes?
periosteal new bone formation
what are the differentials?
seronegatives -erosive hypoparathyroidism -has medullary sclerosis Acromegaly CPPD -linear calcifications
diagnosis of DISH?
made only with axis studies, thorocolumber
most probable when looking at foot
